Dave Jacobs

Co-Founder & Co-CEO

About Dave

Dave Jacobs has extensive experience as a leader in the healthcare industry. His comprehensive knowledge of geriatric care, combined with his passionate advocacy for senior in-home care, has driven him to co-found Homethrive.

Dave spent fifteen years as a senior executive at Medline Industries, a $12 billion healthcare products company. While at Medline, he was President of the Durable Medical Equipment division and ran Medline’s Post-Acute business that encompasses nursing homes, home health, assisted living, and managed care insurance. While in those roles, Dave developed a deep, first-hand appreciation for the benefits of seniors living independently at home and the many challenges seniors and their family caregivers face.

Prior to that, Dave co-founded a technology company that quickly soared to the 77th spot on the Inc. Magazine Fastest Growing Private Companies list. Dave’s varied professional background also includes a position with a private equity company, and an executive level marketing role with the Quaker Oats Company. He currently holds a position on the Board of Directors of a $500m privately held consumer products company.

Dave takes great pride in balancing his work with his busy family life. He lives in Highland Park, IL with his wife and two children.

David Greenberg

About David

As a veteran executive, David has enjoyed a distinguished career rooted in leading multi-billion dollar companies. Prior to co-founding Homethrive, David served as Executive Vice President of Strategy and Group President at Medline Industries, a $12 billion healthcare products company. In his role as Executive Vice President of Strategy, his responsibilities included defining and supporting strategic priorities, leading , business development initiatives to strengthen the Medline portfolio, and M&A. As Group President, David ran Medline’s distribution business (~$5B), and several manufacturing and marketing divisions ($500M+).

Prior to his time at Medline, David also held several senior leadership positions at Aon Corporation which included serving as the CFO of a multibillion-dollar business unit, CEO of a $1B+ Auto Finance division, and managing AON’s ~$2B Private Equity Portfolio. David previously worked as a Corporate Attorney in Chicago, IL. Currently, David holds the position of Lead Director of ICU Medical, a medical device manufacturing company (ICUI: $5B market cap).

David continues his involvement with several charitable organizations and lives in the Chicago suburbs with his wife and three daughters.

David Greenberg earned a BA from the University of Illinois, Urbana Champaign and a JD from the Chicago-Kent College of Law at the Illinois Institute of Technology.

Shanna Grayson

VP, Care Management

About Shanna

At Homethrive, Shanna brings an expansive breadth of healthcare expertise and professional management experiences from her accomplished career in social work to oversee all care management protocol and operations. Prior to joining Homethrive, she was the founding manager of the Care Partner Institute at Nevada Senior Services where she led a team of social service professionals by providing a multitude of evidence-based education and support programs for family caregivers and the Aging and Disability Resource Center.

Shanna is a certified RCI REACH Interventionist through the Rosalynn Carter Institute and worked intimately with caregivers of people living with dementia, teaching them the importance of self-care, stress management, and fundamental coping skills for empowerment and enhancement of overall well-being. Her diverse toolbox of skills includes coaching, advocating, and empowering family caregivers through evidence-based programs and leading social service professionals who share her passion of supporting family caregivers. Early in her career, Shanna worked as a case manager in hospice, skilled nursing, acute care, and other fields within the non-profit sector.

Outside of her professional life, Shanna loves traveling with her family and doting on her high-maintenance dog.

Shanna Grayson earned a Bachelor of Social Work from the University of Nevada and a Master of Social Work from Simmons University.

Leland Brewster

VP, Product

About Leland

At Homethrive, Leland is responsible for leading the development and growth of Homethrive's suite of products serving families, clients, and Care Guides, ensuring that all our members can receive the right support at the right time in order to reduce their work, worry, and stress.

Prior to joining Homethrive, Leland spent four years at Healthbox as a member of the Intermountain Healthcare Innovation Fund investment team where he led the Fund’s investments in Redox as well as Circulation, (acquired by Logisticare in September 2018), and held board observer seats at both companies. Leland also managed Healthbox’s portfolio of 45 active investments from its accelerator programs, supporting the companies’ fundraising, business development, and M&A efforts.

Previously in his career, Leland worked for Clarion Healthcare, a boutique life sciences consultancy in Boston, where he was involved in multiple product commercialization and launch readiness projects.

Leland earned a BA in Economics and Chemistry from Williams College and an MBA from the University of Chicago Booth School of Business.

Alex Gundry

Vice President

About Alex

Alex is responsible for leading a variety of strategic initiatives at Homethrive and brings over 10 years of business experience across a variety of roles.

Most recently, Alex worked at Medline Industries in Product Management where he directly managed a product category of over $105 million within the Advanced Wound Care division. Prior to Medline, Alex spent 3 years at S.C. Johnson managing a variety of brands including Glade and Windex. Previously in his career Alex was in financial services - he worked in Investment Banking at Wells Fargo Securities and in Private Equity at Weston Presidio.

Alex earned a BA in Economics from Connecticut College and an MBA from the Kellogg School of Management at Northwestern University.

Chief Product & Marketing Officer

About Karan

Karan oversees all activities in Homethrive’s marketing and product departments and is responsible for the planning, development, and execution of innovative marketing strategies and initiatives. He ensures that Homethrive’s brand promise and message are consistently delivered across channels and to targeted audiences to meet growth goals, ensuring that Homethrive can support as many caregivers and seniors as possible.

Pat Twohig

Chief Revenue Officer

About Pat

Pat is responsible for Homethrive’s growth efforts, with a heavy focus on overseeing employer sales, partnership development, and client success. He is an experienced and highly accomplished executive with a proven track record of leadership, building and developing teams, and catalyzing growth for health and wellness companies.  

Pat has 20 years in the post-acute care space and is a passionate senior care advocate. He holds a BA from the University of Charleston and a certificate of gerontology from Ohio State and a CEAL (certified executive for assisted living) certification from Ohio Health Care Association.

Pat is very family oriented. He loves his wife and his favorite hobbies would include attending his daughter's travel softball games and his son’s basketball games, along with playing with his two English labs dogs.

Abbas Mahyari

VP, Engineering

About Abbas

Abbas is responsible for leading all engineering activities including product development, data analytics, cloud infrastructure and IT systems, security, and compliance. He ensures Homethrive’s technology stack meets the industry's highest standards to serve families who struggle to support their loved ones at home.

Abbas has over 20 years of experience working at various high-tech startups and large corporations in Silicon Valley building innovative products in content delivery, enterprise security, and business automation. He worked for five years at Limelight Networks as an Engineering Manager to build its mobile content delivery platform and similarly five years at Citrix as a Senior Engineering Manager to build its endpoint management product. He has extensive experience in building global engineering teams, cloud services and SaaS products. Most recently, he worked at SKAEL as Vice President of Engineering to build and launch an AI SaaS platform for workforce automation.

Abbas earned his M.Sc. in Computer Science from McGill University in Montreal, Canada. He holds several patents in AI systems, mobile security and cloud delivery. He lives in the San Francisco Bay Area with his wife and twin kids.

I cared for my mom in my home in her final stages of cancer. This shaped my career path and continues to play a role in every decision I make around care management here at Homethrive.
