Information That You Provide. We collect personally identifiable information about you which you affirmatively choose to submit to us, including your name, date of birth, e-mail address, postal address, phone number, gender, ethnicity, and marital status. As a part of our Services, we may also gather personal health information that you provide to our Care Guides. Personal health information may be medical or behavioral in nature, or a combination of both. From time to time, we may automatically collect certain information that may be considered personally identifiable information, such as the IP address of your computer or network connection. We may also collect information from you during phone or video calls, or via email correspondence with a Homethrive representative.
We also collect content that you create or upload when using the Services. This may include, but is not limited to, pictures, care plan documents (or other personal documents that you choose to upload), calendar reminders, and services requests to be performed by a third-party provider.
Payment Processing Information. When you make a payment in connection with purchasing any of the Services, Homethrive may obtain your credit card, ACH, and/or other payment information.
Usage Information. We may collect other non-personally identifiable information such as the time and date of your visit, your location, the pages and other content that you access, and the number of times you return to the website. This information is used in the aggregate only for our internal marketing and similar purposes. We may also integrate into the website technology provided by third parties (e.g., Google Analytics) to accomplish this information collection. In addition, we may use a variety of other technologies that collect similar information for security and fraud detection purposes.
Partner Information. Homethrive partners with certain third-party vendors to offer you some services, and we may receive certain information directly from those third parties.
We use the information that we collect to provide you with our Services and to coordinate your requests for related third-party vendor services. We use your information as necessary to facilitate communication between you and us regarding the Services, as well as to improve our site and services, to learn about how it is used and accessed, and to offer new features. As necessary to provide the Services, we share pertinent personal information with select affiliates and partners for the purposes of customer support, account management, and similar. We do not sell, trade, share, or rent your personal information provided by you to us through our website to third parties who provide no affiliated services or support.
Homethrive will only use your payment processing information with your explicit consent.
We may disclose information about you to third parties if we have a good-faith belief that we are required to do so by law or legal process, to respond to legal claims, or to protect our rights, property, or safety, or the rights, property, or safety of others. We may disclose information about you to a successor organization in the event of a merger, acquisition, bankruptcy, or other sale or disposition of all or a portion of our assets. The successor organization’s use and disclosure of your personal information will continue to be subject to this policy unless (i) a court orders otherwise, for example a bankruptcy court; or (ii) the successor organization gives you notice that your personal information will be subject to the successor organization’s own policy, along with an opportunity for you to opt out (which may cause you not to be able to continue to use the website). If you submit personal information after such a transfer, that information may be subject to the successor entity’s policy.
We keep your information as long as it is needed to provide you the Services described in this policy. When we no longer need to use your information and there are no legal or regulatory compliance obligations, we will either remove it from our systems or anonymize it to remove your identity.

No method of transmission over the Internet, or method of electronic storage, is 100% secure. Therefore, we cannot guarantee the absolute security of your personal information. We realize there can be incidents of misuse or unauthorized program incursions, as almost every website, service and user encounters. In those instances, our goal is to move quickly to isolate the problem, ensure or restore proper functionality and minimize any inconvenience to our Users. As appropriate and necessary, Homethrive will notify the relevant authorities of these incidents of misuse or unauthorized program incursions of the Homethrive website.
Homethrive’s services and features are not intended for children, and we do not knowingly collect any personal information from children under 13 years of age.
California residents have additional rights regarding Homethrive’s use of your personal information, including the right to give the instruction Do Not Sell My Personal Information. Other than as expressly set forth herein, Homethrive does not sell or otherwise share your personal information with third parties.
California residents are also entitled once a year, free of charge, to request and obtain certain information regarding our disclosure, if any, of certain categories of personal information to third parties for their own direct marketing purposes in the preceding calendar year. Homethrive does not share personal information with third parties for their own direct marketing purposes.
Homethrive is operated in the United States. If you are located outside of the United States, please be aware that information we collect will be transferred to and processed in the United States. By using the Homethrive website or the Services, or providing us with any information, you consent to this transfer, processing, and storage of your information in the United States.
